Exposure and Response Prevention (ERP) is a type of cognitive behavioral therapy that is considered one of the most effective treatments for obsessive compulsive disorder (OCD). ERP is based on the understanding that OCD is maintained through a cycle of intrusive thoughts, anxiety, and compulsions. When a person performs compulsions such as checking, reassurance seeking, avoidance, mental reviewing, or rituals, they may feel temporary relief, but the brain learns that the obsession must have been important or dangerous in the first place, which can keep that cycle going.
ERP helps treat OCD by gradually and intentionally facing the thoughts, situations, images, sensations, or uncertainties that trigger distress while resisting the urge to perform compulsions. Rather than trying to eliminate intrusive thoughts completely, therapy focuses on learning how to tolerate the uncertainty, discomfort, and anxiety without relying on rituals or avoidance for relief. Over time, this can help reduce the intensity and frequency of obsessions while increasing confidence in your ability to handle distress without compulsions.
